Identification:
Valid driver’s license or state ID for each adult on the return
Social Security card for each adult and child on the return
Income Documentation:
All W-2’s (incl. gambling/lottery winnings) All 1099’s (incl. Socal Security, pension, unemployment, interest/dividends, misc. income)
DHHS Assistance (annual statement) Child Support (annual statement)
Alimony amounts received
Letter from Social Security for SSI Income 1095’s for Market Place Medical Insurance ****& Employer Insurance
Past Years Completed Tax Returns
Expenses & Other Documents:
Any and all childcare expenses
Property tax statement
Mortgage interest statement
Monthly rent amount (if applicable)
January gas bill for heating credit
Student loan interest or tuition and course materials
Itemized car insurance policy that shows PIP (Personal Injury Protection) coverage
Direct Deposit Information:
Bank routing number and account number, checking or savings (Debit cards do not have this information on them but checks do)
